Staff Nurse - LPN

Overview Of Position

The Nurse is an essential member of the patient center medical home team. Responsible for managing the individualized patient care, by promoting and restoring patient’s health through the nursing process; collaborating with provider and multi-disciplinary team members; provides physical and psychological support.

Education And Experience

• Diploma in nursing with 2-years’ experience
• Current licensure through South Carolina State Board of Nursing
• CPR (Cardiopulmonary Resuscitation) or BLS (Basic Life Support) certification required
• Possess excellent customer service skills and be well organized
• At least 2 years experienced working in a primary or specialty care medical practice
• Ability to work in a high-pressure environment

• Must possess excellent oral and written communication

• Knowledge of policies, procedures, regulations, and protocols
• Knowledge of data bases (ex: EMR systems)
• Bilingual in English/Spanish preferred
• Maintain current medical knowledge to include pharmacology ( medication names and usage) knowledge required

Essential Job Functions

• Assess the needs of patients based on data collected through history observation, physical exam, telephone triage, and analysis or diagnostic data
• Performs direct patient care: triage, assessment, procedures, treatment, patient education, and follow up instructions
• Aid patients and providers in obtaining medicinal prior authorizations when required
• Verifies the patient understands the purpose of the visit. Providers preventative Care information matching the patients age, gender, and health status
• Maintain knowledge of the principles of the skills needed for practical nursing to provide patient care and treatment
• Knowledge of examination, diagnostic and treatment room procedures
• Ability to interpret, adapt, and apply guidelines and procedures

Basic Critical Care and Chronic Illness Management

• Required to participate in training opportunities
• Helps promote office efficiency and encounter volumes
• Assists in training new nurses to the organization in the standard working procedures of HopeHealth

Physical Requirements

• Hearing: Adequate to perform job duties in person and over the telephone
• Speaking: Must be able to communicate clearly to patients in person and over the telephone
• Vision: Visual acuity adequately to perform job duties, including reading information from printed sources and computer screens
• Other: Requires occasional lifting and carrying items weighing up to 10 pounds unassisted in normal daily activities and up to 30 lbs. for other required work activities. Additionally, requires frequent bending, reaching, and repetitive hand movements (especially if keyboarding and writing), standing, walking, squatting, and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular work shift.
