Staff Nurse, Ambulatory- Primary Care

The Ambulatory Registered Nurse supports patients through clinical triage, care coordination, patient education, medication refill management, preventive‑care follow‑up, and ongoing communication to create a seamless care experience.

Responsibilities

• Perform clinical telephone triage and support patients with acute concerns, chronic condition management, and preventive‑care needs.
• Coordinate care and follow‑up, including care‑gap closure.
• Provide patient education and counseling.
• Manage medication refill requests.
• Facilitate communication among patients, providers, and insurance processes.

Requirements

• Graduate of an accredited ASN or BSN nursing program.
• Current Pennsylvania RN license.
• Provider BLS certification (American Heart Association or American Red Cross).
• Preferred: Experience in ambulatory clinical nursing, primary care, family medicine, internal medicine, or outpatient office settings.
• Strong telephone triage experience and comfort supporting patients with acute and chronic conditions.
• Experience with medication refills, care‑gap closure, patient education, care coordination, and insurance‑related processes.

Schedule

40 hours per week, day shift. Monday through Friday, 8:00 AM – 4:30 PM. No weekends or holidays.
