About the Role
The Staff Development Nurse promotes optimum health and wellbeing for adults with intellectual and developmental disabilities in group home and apartment settings. This role acts as a health advocate, supports individuals and interdisciplinary teams, and ensures compliance with state and federal regulations (245D). The nurse provides training and guidance to staff on medical procedures, device usage, documentation, and incident reporting, while also serving as a resource for health-related concerns.
Responsibilities
- Monitor health status, particularly urgent/emergency situations and medication administration needs.
- Report changing health conditions to appropriate team members and triage concerns promptly.
- Arrange medical/remedial services and incorporate recommendations into Support Plans.
- Assess and address changes in physical, behavioral, or mental health status.
- Evaluate staff competency in health-related tasks and provide retraining as needed.
- Document assessments, observations, and interventions according to standards.
- Educate individuals, families, and staff on health, disease, and self-responsibility.
- Develop and facilitate medical training for staff (orientation, medication administration, continuing education).
- Respond to nurse emergency calls on a rotating basis.
- Attend mandatory staff meetings and in-services.
- Coach staff on dietary needs and provide hands-on training for vital signs, medication administration, etc.
- Train staff on medical device usage (blood pressure cuffs, glucometers, nebulizers, etc.).
- Educate staff on recognizing and reporting health status changes.
- Perform skill refreshers and competency assessments for staff.
- Advocate for the needs of developmentally disabled individuals in the community.
- Ensure compliance with HIPAA, PHI, and infection control protocols.
- Collaborate with physicians, pharmacies, therapists, and support coordinators.
- Serve as backup nursing support when needed.
- Process paperwork accurately and promptly.
Qualifications
- Licensed as an LPN or RN in good standing with the State of Minnesota.
- Proficiency in managing health services for individuals with intellectual disabilities.
- Valid driver's license meeting Hammer Residences insurance requirements.
- MN DHS Background study clearance and negative TB test result.
- Excellent written/verbal communication skills and Microsoft Office proficiency.
- Strong organizational skills and ability to manage multiple projects.
- Commitment to confidentiality, customer service, and ethical standards.
- Experience working with diverse individuals and teams.
Physical Demands
- Frequent telephone and computer use.
- Ability to perform nursing assessments, evaluations, and skilled tasks.
- Lifting up to 40 pounds, pushing/pulling wheelchairs, and assisting with transfers.
- Operating specialized medical/adaptive equipment and driving a lift van.

Typical Schedule: Primarily M-F days with flexibility for occasional evenings/weekends. Emergency call rotation required. Work conducted across two offices and various programs.
