Staff Clinical Pharmacist

About the role

We are seeking a Staff Pharmacist responsible for delivering patient-centered pharmaceutical care, including clinical services such as antibiotic and renal dosing, warfarin management, and IV-to-PO conversions.

Responsibilities

• Preparing IV admixtures
• Entering and verifying medication orders
• Participating in patient rounds
• Collaborating with healthcare teams to ensure safe and effective medication therapy
• Evaluating drug regimens
• Providing medication education
• Supervising pharmacy personnel

Qualifications

• Bachelor's or Doctorate in Pharmacy
• Licensure as a Registered Pharmacist
• Board Certification in Pharmacotherapy (preferred)
• Previous hospital pharmacy and clinical experience (preferred)
