Priorities and Responsibilities
- Prioritize and plan visits based on client condition, physician orders, and available resources.
- Maintain knowledge of laws, regulations, ethics, and agency policies.
- Audit clients at admission and required intervals using standardized documentation.
- Develop and revise treatment plans with physician and client, including attainable goals.
- Make referrals and coordinate care with other disciplines.
- Apply professional judgment in evaluations and interventions.
- Implement preventive, therapeutic, and rehabilitative procedures per treatment plan.
- Educate clients and families on communication and swallowing improvement.
- Report changes in client condition promptly and obtain updated orders.
- Participate in quality improvement and peer review.
- Document all care accurately and timely in the point-of-care system.
- Ensure safe home environment and discharge planning.
Qualifications
- Mets the educational and experience requirements for a Certificate of Clinical Competence in Speech Pathology granted by the American Speech and Hearing Association; or, meets the educational requirements for certification and is in the process of accumulating the supervised experience required for certificate.
- Registration or license to practice as a speech pathologist in the state of practice if applicable.
- Accepts responsibility for maintaining clinical practice skills, learning and adhering to agency’s policies and procedures on an on-going basis.
- Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with the agency’s clients and their families.
- Current Driver’s license and reliable transportation to and from work site.
- Accurately documents observations, evaluations and interventions pertaining to client care management and services in accordance with agency policy and procedure utilizing the point of care information system.
- Takes appropriate steps to assure timely submission of client data to the office for inclusion in the client record (by timely electronic transmission of data, or when appropriate by fax or mail).
- Works with scheduling to assure visits and client care coverage are scheduled in accordance with the plan of care and plans ahead for PTO or other time off.
- Ability to consistently demonstrate commitment to the mission and Organizational Code of Ethics and adhere to the Compliance Program.
Physical & Mental Requirements
- Occasional lifting, standing, walking, bending, and positioning patients.
- Ability to manage detailed tasks with frequent interruptions.
- Adaptable to changing work parameters; travel required.
