About the role

The SSVF Healthcare Navigator (HCN) at Recovery Resource Council is responsible for connecting Veterans to VA health care benefits or community health services where Veterans are not eligible for VA care. This role requires a combination of case management, care coordination, and collaboration with the Veteran's primary care provider and interdisciplinary treatment team.

Responsibilities

  • Conduct assessments with Veteran clients to understand their situation, potential barriers to care, the causes, and the impact of barriers on the Veteran's ability to access and maintain health care services, as well as their strengths, limitations, risk factors, and supports.
  • Work with the interdisciplinary team to develop a care plan.
  • Provide care coordination and connection with services from other organizations and programs to assure services are complementary and comprehensive.
  • Evaluate the effectiveness of resources and referrals and make modifications as needed.
  • Maintain thorough documentation and stay in communication with the treatment team members when appropriate.
  • Support the veteran to meet their health goals and provide ongoing follow-up.
  • Provide health education and materials to the Veteran and family members.
  • Work with and interact with Veterans from a variety of ethnic, cultural, and economic backgrounds, adhering to the agency guarantee to provide everyone with "unconditional acceptance, respect, and positive regard."

Requirements

  • Bachelor’s degree in a related field, with a Master’s degree in Social Work or equivalent preferred.
  • Experience working with people experiencing homelessness and/or veterans is preferred.
  • Reliable transportation, current Texas driver’s license, and evidence of auto liability insurance.
  • Ability to work one-on-one or with large groups, work independently and as part of an interdisciplinary team, plan, organize, set objectives, time frames, and prioritize.
  • Ability to write clear, concise case notes and strong communication skills both orally and in writing.
  • Knowledge of substance use disorders and mental health issues, community resources, and ability to work flexible schedules.
  • Understanding of and willingness to practice trauma-informed care.

Qualifications

  • Ability to conduct client assessments and formulate client goals and objectives.
  • Knowledge of community resources and ability to network.

This is a hybrid position requiring both remote and in-person work, involving sitting, standing for various lengths of time, and frequent driving for long distances.
