Sr. Population Health Specialist

Job Responsibilities

• Acts as program/project coordinator for assigned initiatives.
• Affirms leadership in the execution, evaluation, planning, & coordination of programs/projects' short/long-term goals to promote & achieve PHHEs goals within communities & healthcare systems.
• Identifies & use appropriate project management tools & concepts (charters, scope management, budget management, risk/issue management, communications management).
• Sets priorities for assignments based on importance & urgency.
• Tracks progress & prepares internal reports for Center leadership, CCHMC institutional needs, & community facing reports.
• Prepares & distributes project/program communication to internal & external stakeholders.
• Identifies people, process, & infrastructure needs & act as liaison to ensure appropriate systems & reporting are maintained in support of project/program requirements.
• Understand & meet expectations for deliverable quality & timeliness.
• May lead research inventory across departments' engagement to build new grant mechanisms.
• Assess, prioritize, & allocate resources to support equity research - such as data, grant review processes, & grant writing support.
• Serves as the primary liaison for daily administration and operational issues such as data management, marketing/communications, outreach, annual conference coordination, and social media coordination.
• Develop and implement policies and procedures for improved utilization and quality of programs, projects, and operations.
• Supports yearly operating budgets for projects and programs.
• May lead and provide training and guidance to team members.
• Aid leadership with the allocation of resources and staff for assignments.
• Delegate responsibilities to reporting staff and establish clear lines of responsibility and accountability.
• Participate in direct reports' recruitment, onboarding, training, and performance appraisal processes.
• Collaborates with quality improvement specialists and data analysts to develop innovative aims, measures, strategies, milestones, and goals for efficient data management.
• Identify methods to increase data collection, quality, and utilization to streamline and enhance reporting processes and deliverables.
• Supports leadership and stakeholders to develop metrics around clinical equity, community health needs, challenges, and operational resources to reduce health disparities.
• Show strong ability to pursue interdisciplinary collaboration.
• Comfortable and willing to engage with community members and organizations.
• Demonstrates emphasis on relationship building / relational work, flexibility, and adaptability to change.
• Open to continuous learning and capable of taking a systems-learning view.

Job Qualifications

• Bachelor's Degree and 3+ years of work experience
• Master's Degree and 2+ years experience
