About the role

Supporting older adults in maintaining independence and quality of life.

Responsibilities

  • Provide comprehensive care services including medical, social, and rehabilitative services.
  • Collaborate with healthcare providers to develop and implement individualized care plans.
  • Facilitate access to community resources and support networks.
  • Ensure compliance with regulatory standards and policies.

Requirements

  • Bachelor’s degree in healthcare, gerontology, or related field.
  • Experience in healthcare administration or geriatric care preferred.
  • Strong interpersonal and communication skills.
  • Ability to manage multiple tasks and prioritize effectively.

Qualifications

  • Valid driver’s license and reliable transportation.
  • Proficiency in Microsoft Office Suite.
  • Knowledge of PACE program principles and operations.

  • Strong written and verbal communication skills.

Pay

Salary range: $60,000 - $80,000 annually.

Schedule

Full-time position, Monday through Friday, with some evening and weekend availability required.
