Primary Responsibilities And Essential Functions Of Position
- Oversight of DSP implementation of individuals’ plans, consistent with their specific services, goals, and outcomes.
- Participation in the planning and implementation of activities, both at the program and in the community, that address these goals and outcomes.
- Promotion of choice and self-determination to support individuals to direct the course of their own lives.
- Ensuring that services and goals reflect what the individuals want for themselves.
- Ensuring DSPs support and teach individuals according to their services, goals, and outcomes.
- Active engagement in skills development, socialization, and task participation across all settings.
- Ensuring that supports are provided with consideration for each person’s individualized needs and routines.
- Promotion and support of the inclusion of individuals in their community according to their unique needs, interests, and desires.
- Modeling and encouraging interaction between individuals and community members.
- Fostering individuals’ personal growth by assisting in forming and maintaining friendships with neighbors, co-workers, and peers.
- Aid in maintaining positive communications with family members, advocates, and service providers as needed.
- Acting as an advocate for individuals.
- Transporting individuals utilizing agency vehicles or assisting them in taking public transportation, as required.
- Planning monthly schedule with team for community outings and in-house activities.
- Being prepared each day to go into the community with individuals and overseeing the DSPs in the community.
- Participating in program planning meetings.
- Creating staff action plan with the team, putting plan into review in Precision Care, and submitting to supervisor for approval.
- Reviewing meeting notes and life plan to ensure the goals are consistent.
- Sending copies of the staff action plan to care manager, residence, and family within the required timeframe.
- Uploading all meeting documents, including sign-in sheet, meeting minutes, and proof of plan dissemination, into Precision Care.
- Training all staff on plan and documenting training.
- Documenting and ensuring documentation of necessary information and data in areas such as medical, behavioral, individualized services, goals and outcomes, fire safety, community inclusion, communication books, etc., ensuring their accuracy and completeness.
- Completing all monthly summaries by the 10th of every month.
- Ensuring all required documents in individuals’ program books are up to date and uploaded into Precision Care.
- Providing appropriate levels of supervision on an individualized basis for staff and individuals, ensuring staff are trained on the LOS for each person.
- Actively engaging in all program services as needed, including toileting, behavioral interventions, community inclusion, emergencies, physical plant cleanliness, etc., as needed and required.
- Reporting all incidents to program management immediately upon discovery.
- Immediately safeguarding all individuals and assisting with the completion of paperwork as required.
- Aiding in securing safeguards, protective measures, and notifications for incidents.
- Ensuring all occurrence reports are reviewed, completed accurately, and given to the program nurse or coordinator the same day as the occurrence.
- Ensuring that equipment and supplies are appropriate, in good condition, and safely stored.
- Overseeing cleaning of program area and ensuring cleaning is completed as frequently and thoroughly as necessary to meet cleanliness standards.
- Reporting any hazards or unsafe conditions immediately to supervisor.
- Undertaking any additional tasks and initiatives within the scope of the role as per the specific needs of the programs and individuals and as assigned by supervisor.
Qualifications
- High School Diploma or GED required; bachelor's degree from four-year college or equivalent preferred.
- Valid NYC Driver’s License required.
- Minimum 2 years in the field with experience in supervision and paperwork.
- Minimum of 2 years or more experience working with individuals with developmental disabilities.
- Prior staff supervision experience strongly preferred.
