About the role
Provides direct spiritual care and counseling to hospice patients and their families, collaborating with the interdisciplinary team (IDG) to ensure a meaningful and comfortable end-of-life experience. Works under clinical supervision, implementing the spiritual care program in accordance with CMS standards.
Responsibilities
- Assess emotional, spiritual, religious, and existential needs of patients and families.
- Deliver spiritual counseling, crisis intervention, and develop care plans aligned with patient/family goals.
- Utilize community resources and serve as liaison with clergy.
- Document assessments, interventions, and care plans in EMR, collaborating with the IDG.
- Participate in team meetings, develop treatment strategies, and support interdisciplinary collaboration.
- Serve as a resource for staff regarding psycho-spiritual and religious factors affecting quality of life.
- Travel to patient homes or facilities to provide direct spiritual care and participate in special projects.
- Maintain professional growth and provide on-call services as needed.
Qualifications
- Valid driver’s license required.
- Bachelor’s degree in Humanities or related field; Master’s in Divinity or Theology preferred.
- Four units of Clinical Pastoral Education (CPE) completed.
- Minimum one year of chaplain experience, preferably in hospice.
- Effective communication skills and proficiency in MS Office required.
- Board Certification as a Chaplain preferred.
