Spiritual Care Counselor - Hospice Home Care

About the role

Provides direct spiritual care and counseling to hospice patients and their families, collaborating with the interdisciplinary team (IDG) to ensure a meaningful and comfortable end-of-life experience. Works under clinical supervision, implementing the spiritual care program in accordance with CMS standards.

Responsibilities

  • Assess emotional, spiritual, religious, and existential needs of patients and families.
  • Deliver spiritual counseling, crisis intervention, and develop care plans aligned with patient/family goals.
  • Utilize community resources and serve as liaison with clergy.
  • Document assessments, interventions, and care plans in EMR, collaborating with the IDG.
  • Participate in team meetings, develop treatment strategies, and support interdisciplinary collaboration.
  • Serve as a resource for staff regarding psycho-spiritual and religious factors affecting quality of life.
  • Travel to patient homes or facilities to provide direct spiritual care and participate in special projects.
  • Maintain professional growth and provide on-call services as needed.

Qualifications

  • Valid driver’s license required.
  • Bachelor’s degree in Humanities or related field; Master’s in Divinity or Theology preferred.
  • Four units of Clinical Pastoral Education (CPE) completed.
  • Minimum one year of chaplain experience, preferably in hospice.
  • Effective communication skills and proficiency in MS Office required.
  • Board Certification as a Chaplain preferred.
