Speech Therapist (SLP) - Home Health

About the Role

Transition Home Healthcare is an elite home health agency serving Houston, TX and surrounding areas since 2010. We are a group of passionate people dedicated to delivering the highest quality services to our patients, families, team members, and the community. Our team is growing and looking for an experienced Speech Language Pathologist.

Shifts Available: PRN

Service Areas with Openings: Northeast, Northwest, Southwest, Southeast

Responsibilities

• Facilitates rehabilitation of patients with emotional, physical, or mental disabilities.
• Implements speech therapy care plans.
• Coordinates care with entire team.
• Educates patients and family members.
• Prepares timely, complete, and accurate clinical notes.
• Communicates effectively with patients, caregivers, healthcare providers, office support and other care team members.

Qualifications

• Speech language pathologist in the State of Texas.
• Home health experience highly preferred.
• Bilingual (Fluent in Spanish) encouraged to apply.
• Experience with Home Care Home Base EMR (preferred but not required).
• Knowledge of home health regulations and general healthcare compliance.
• Ability to handle physical workload.
• Good driving record.
• Proficient level experience in Microsoft Office (Word, Excel, PowerPoint, Outlook).
• Exceptional communication and people skills.
