Speech Therapist - PRN Home Care

Position Overview

Key Responsibilities

• Performs specialized assessments
• Coordicates referrals and plans for discharge to community resources
• Maintains communication with supervisors, healthcare teams, and caregivers regarding patient care
• Participates in multidisciplinary teams, community outreach, and ongoing education to enhance professional growth
• Affirms patient environments, recommends adaptive equipment or modifications, and manages durable medical equipment procurement
• Develops and modifies individualized treatment plans based on assessments, including dysphasia, oral motor, and language evaluations
• Documents services per standards and regulatory requirements, and educates patients/caregivers about treatment and care management
• Delivers speech-language therapy services such as language exercises, oral motor therapy, and thermal stimulation
• Supervises home health aides and collaborates with healthcare providers to ensure optimal patient outcomes

Qualifications & Skills

• Master's degree in Speech-Language Pathology
• 1-2 years of clinical experience
• Licenses or certifications as required for practice, including state licensure or ASHA certification
• Valid driver's license, dependable vehicle, and CPR certification required
• Strong clinical, communication, and basic computer skills
