Responsibilities

• Assessing and developing personalized therapy programs to meet client needs
• Implementing therapy plans and coordinating modifications with clinical staff and physicians
• Ensuring timely documentation and effective communication with clients, families, and healthcare providers
• Maintaining accurate medical records and evaluating therapy effectiveness
• Instructing clients and caregivers to promote optimal functioning
• Addressing client or employee concerns and complaints

Qualifications and Skills

• Current Speech Therapy license in the applicable state
• Valid CPR certification
• Valid driver's license and insurance
• Preferred: Experience in Home Health Therapy and OASIS documentation
