Speech Therapist - Mobile Outpatient

About the role

As a Speech Language Pathologist at Powerback, you will primarily provide outpatient rehabilitation services in an in-home setting for the older adult population. This may include a patient's private residence, assisted living facility, or independent living facility. Powerback Rehabilitation To You allows you to evaluate and treat patients in their most functional environment—their home. This position offers a flexible schedule and work environment across various settings.

Responsibilities

• Supervise CFYs, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards.
• Adhere to department and patient treatment schedules, timely clinical and billing documentation, and work-related meetings.
• Attend and contribute to patient care, staffing conferences, and other related meetings.
• Make referrals to other disciplines and agencies to address the comprehensive needs of the patient.
• Consult with members of the rehabilitation team and physicians to select appropriate therapeutic regimens based on patient needs and capabilities.
• Adapt programs and care plans according to the individual needs of each patient.
• Promote a safe environment and enforce the elimination of fire and safety hazards.
• Order and maintain supplies and equipment, reporting repair needs as necessary.
• Prioritize patient service, ensuring residents and families receive high-quality care in a compassionate atmosphere.
• Screen, examine, and evaluate patients, including history, systems review, and application of appropriate tests and measures.
• Establish diagnoses, identify impairments, and determine treatment plans, goals, and methods in collaboration with patients and caregivers.
• Implement speech-language pathology treatment plans, including therapeutic techniques in areas such as:
  • Cognitive-communication
  • Speech intelligibility
  • Oral motor skills
  • Swallowing disorders/dysphagia
  • Perceptual abilities
  • Orientation, memory, and pragmatics
  • Psychosocial expression
  • Functional communication
  • Case management in the home and community
• Coordinate the acquisition of adaptive communication devices and instruct patients in their use.
• Identify hearing deficits and refer patients for assessment, diagnosis, rehabilitation, or personal amplification.
• Select appropriate therapeutic activities based on the patient's physical and mental capacity, intelligence level, and interests.
• Instruct patients in skills and techniques required for therapeutic activities and evaluate progress, attitudes, and behavior.
• Perform routine reexaminations to modify or progress the treatment plan as needed.
• Coordinate care in the home and community with interdisciplinary team members and referring physicians.
• Perform other related duties as required.

Qualifications

• Valid Speech Language Pathologist license in the state(s) of practice or proof of license eligibility.
• Valid driver’s license or reliable, independent transportation to and from patients' residences.

Pay

USD $41.00 - USD $61.00 per hour.
