Role Overview: The Speech Language Pathologist provides outpatient rehabilitation services in patients' homes or care facilities, focusing on communication and swallowing disorders for older adults. They evaluate, develop, and implement individualized treatment plans, promoting patient independence and health. The role involves coordinating with interdisciplinary teams, supervising support personnel, and ensuring comprehensive patient care. Key Responsibilities Conduct assessments, develop goals, and deliver therapy techniques across areas such as cognition, speech, oral motor skills, and dysphagia.Coordinate adaptive communication devices and refer for hearing assessments as needed.Adapt care plans, monitor progress, and document services accurately.Collaborate with healthcare providers and support staff to ensure quality care in various settings.Maintain equipment, promote safety, and ensure compliance with ethical standards. Qualifications & Skills Valid State License as a Speech Language Pathologist or license eligibility.Reliable transportation or valid driver’s license.Strong communication, patient assessment, and care coordination abilities.
