Responsibilities

The Speech Language Pathologist will primarily provide outpatient rehabilitation services in an in-home setting for the older adult population. The in-home setting could be a patient's private residence or a patient's room in an assisted living or independent living facility. Powerback Rehabilitation To You allows evaluation and treatment in the most functional environment—the patient's home.

• Organize and provide speech-language services to facilitate rehabilitation, directing patient participation in tasks to restore, reinforce, and enhance performance.
• Facilitate learning of skills and functions essential for communication and swallowing, diminishing or remediating disorders or deficits.
• Direct activities to promote and maintain health within the scope of practice and consistent with the Code of Ethics of the profession.
• Supervise CFYs, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards.
• Adhere to department and patient treatment schedules, timely clinical and billing documentation, and work-related meetings.
• Attend and contribute to patient care, staffing conferences, and other related meetings.
• Make referrals to other disciplines and agencies to address comprehensive patient needs.
• Consult with rehabilitation team members and physicians to select appropriate therapeutic regimens consistent with patient needs and capabilities.
• Adapt programs and/or plans of care according to individual patient needs.
• Promote a safe environment and enforce elimination of fire and safety hazards.
• Order supplies and equipment as necessary; clean, maintain, and report equipment repair needs.
• Prioritize patient service, ensuring residents and families receive high-quality care in a compassionate atmosphere.
• Screen, examine, and evaluate patients, including history, systems review, and application of appropriate tests and measures.
• Synthesize information and interpret examination findings to establish diagnoses, identify impairments, and determine predicted levels of improvement.
• Develop treatment goals and methods in collaboration with patients and caregivers, implement speech-language pathology treatment plans, and complete related documentation.
• Provide therapeutic techniques in areas such as:
  • Cognitive-communication
  • Speech intelligibility
  • Oral motor skills
  • Swallowing disorders/dysphagia
  • Perceptual abilities
  • Orientation, memory, and pragmatics
  • Psychosocial expression
  • Functional communication
  • Case management in the home and community
• Coordinate acquisition of adaptive communication devices and instruct patients in their use.
• Facilitate identification of hearing deficits and refer for assessment, diagnosis, rehabilitation, and/or personal amplification.
• Select appropriate activities suited to individual physical and mental capacity, intelligence level, and interest.
• Instruct patients in skills and techniques required for participation in therapeutic activities and evaluate progress, attitudes, and behavior.
• Perform routine reexamination as needed to modify or progress the treatment plan.
• Coordinate care in the home and community with interdisciplinary team members in collaboration with the referring physician.
• Perform other related duties as required.

Qualifications

• Valid Speech Language Pathologist license in the state(s) of practice, or proof of license eligibility.
• Valid driver's license or reliable, independent transportation to and from patients' residences.

Pay

USD $48.00 - USD $63.00 per hour.

Schedule

Part-time, 15 hours per week, weekdays.
