Speech Therapist
About the role
Nascentia Health is seeking a Speech Therapist to provide skilled, compassionate care in home settings. The ideal candidate will have a current New York State license as a Speech-Language Pathologist and at least one year of experience in home care.
Responsibilities
- Conduct comprehensive patient assessments using best clinical practices
- Develop and implement individualized plans of care
- Deliver safe and effective speech therapy interventions in the home setting
- Maintain accurate, timely, and compliant clinical documentation, including OASIS requirements
- Manage a diverse patient caseload while ensuring quality and continuity of care
- Communicate patient status, progress, and care needs with the interdisciplinary team
- Supervise and support paraprofessionals (e.g., home health aides) as it relates to the patient care plan
- Monitor and evaluate the quality of home health aide services
- Ensure adherence to all agency policies, procedures, and regulatory requirements (including HIPAA and infection control practices)
- Participate in case conferences, team meetings, and quality improvement initiatives
- Maintain productivity standards and professional competency requirements
- Engage in ongoing professional development, including in-services and continuing education
- Represent Nascentia Health with professionalism in all patient and community interactions
- Perform other duties as assigned
Requirements
- Current New York State license as a Speech-Language Pathologist required
- Registration with the New York State American Speech-Language-Hearing Association (ASHA) and educational standard board as applicable
- Minimum of one year of experience as a Speech Therapist preferred; home care experience a plus
- Strong clinical assessment and treatment planning skills
- Excellent communication and interpersonal skills
- Ability to work independently in a field-based setting while collaborating with a team
- Strong organizational and documentation skills
- Knowledge of federal, state, and local healthcare regulations
- Current New York State drivers license and reliable transportation with automobile insurance required

Pay
The full-time position is an S03 exempt position with a min-max rate of: $75,250 -$87,138.92 annually.
Schedule
Monday-Friday, Daytime Schedule
Qualifications
Physical Requirements: Ability to sit or stand for long periods during work day; ability to perform tasks involving physical activity (including but not limited to): Pushing/pulling on average 50 lbs. with a max of 150 lbs.; lifting/carrying on average 25 lbs. with a max of 50 lbs.; patient transfers, extensive bending, standing, kneeling, walking in various weather conditions and ability to go up and down stairs. Ability to drive in ALL weather conditions (rain, sleet, snow, wind). Visual/hearing ability sufficient to comprehend written or verbal communication. Ability to use telephone and communicate over the phone. Frequently uses a laptop computer to enter orders and obtain product information. Bloodborne Pathogens Exposure Category I - Employee has tasks that routinely involve exposure or potential exposure to blood, body fluids or tissues.
