Speech Language Pathologist, SLP

BAYADA Home Health Care is seeking a per diem Speech Language Pathologist, SLP for home health visits in Raleigh-Durham Senior Living offices in Wake and Durham counties.

Responsibilities

• Make home visits to clients in designated territories.
• Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for speech, language, communication, or swallowing difficulties.
• Develop a treatment plan based on evaluations, physician’s orders, and recommended goals.
• Continually assess and revise the speech therapy care plan, participating in the multidisciplinary care plan as appropriate.
• Educate and instruct patients, family members, or other patient representatives on rehabilitative care and activities necessary for health, safety, and independent living.
• Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.

Qualifications

• A current license as a Speech Language Pathologist in North Carolina.
• A certificate of clinical competence in speech from the American Speech and Hearing Association, or equivalent educational requirements and work experience.
• Prior experience with electronic medical records (EMR) preferred.
• Strong computer skills.
