Responsibilities
- Evaluate residents referred for speech-language pathology services
- Assess speech, language, communication, cognitive, and swallowing disorders
- Develop individualized treatment plans based on each resident's needs, abilities, and rehabilitation goals
- Provide direct therapy to improve communication, cognition, and swallowing function
- Establish measurable and clinically appropriate treatment goals
- Maintain accurate and complete clinical documentation
- Monitor resident progress and modify treatment plans as needed
- Complete evaluations, progress notes, recertifications, and discharge documentation accurately and on time
- Collaborate with physicians, nurses, physical therapists, occupational therapists, and other members of the interdisciplinary team
- Perform other duties as assigned by the Director of Rehabilitation
Qualifications
- Master's degree in Speech-Language Pathology from an accredited college or university
- Current state licensure as a Speech-Language Pathologist
- Certificate of Clinical Competence (CCC-SLP) preferred
- Experience in skilled nursing, rehabilitation, long-term care, or geriatric care preferred, but not required
- Strong communication, clinical, and interpersonal skills
- Ability to work effectively within an interdisciplinary team
- Commitment to compassionate, resident-centered care
