Evaluate Patients

Evaluate patients for speech and language disorders, initiate assessments upon referral, and develop individualized plans of care in collaboration with the physician and multidisciplinary care team.

Provide Therapy Services

  • Provide rehabilitative speech-language therapy services, carry out appropriate interventions, and educate patients, families, and caregivers on treatment procedures and supportive activities.
  • Maintain compliance with agency policies, patient rights, confidentiality, infection control, safety standards, and emergency preparedness while managing workload, scheduling, and priorities effectively.

Document Progress

Monitor, document, and report patient progress, responses to therapy, and changes in condition using the agency’s Problem Oriented Record (POR) and required clinical documentation systems.

Coordinate Care

  • Coordinate care with healthcare team members,
  • Consult with supervisors,
  • Participate in case conferences, in-services, and ongoing professional development.

Qualifications

  • Active, good-standing MA Speech-Language Pathologist license
  • 1-3 years of experience in geriatrics, long-term care, or short-term rehab
  • Maintains professional and educational competency in compliance with state and federal regulations
  • Physically able to perform job duties, including standing, sitting, walking, bending, squatting, and lifting/carrying up to 20 lbs unassisted and up to 50 lbs with assistance
  • Demonstrates strong interpersonal skills, effectively interacting with residents, families, visitors, government personnel, and interdisciplinary staff
