As a Home Health Speech-Language Pathologist (SLP) with Interim HealthCare, you’ll provide personalized, one-on-one therapy services to patients in the comfort of their own homes, where care is individualized and outcomes are meaningful.
Responsibilities
- Provide home-based speech therapy services to patients experiencing communication, cognitive, or swallowing disorders due to illness, injury, or medical conditions
- Conduct patient evaluations and develop individualized treatment plans based on patient needs and goals
- Assess speech, language, cognitive, and swallowing abilities
- Implement therapeutic exercises and interventions focused on improving communication, cognition, fluency, articulation, and swallowing function
- Maintain accurate and timely clinical documentation
- Educate patients and family members regarding therapy goals, treatment plans, safety practices, and home exercise programs
- Collaborate with physicians, nurses, therapists, caregivers, and other healthcare professionals to support patient outcomes
Requirements
- Master’s or Doctoral degree in Speech-Language Pathology
- Current Speech-Language Pathologist (SLP) license in the State of New Hampshire
- Minimum of one (1) year of speech therapy experience preferred
- Home health experience preferred, but not required
- Current CPR certification required
Qualifications
- Strong communication, clinical assessment, and interpersonal skills
- Caregiver-friendly and patient-focused approach to care
- Knowledge of state and federal home health regulations preferred
