About the role

The Speech Language Pathologist (SLP) provides direct speech therapy rehabilitation care to patients, supporting their functional progress and improving quality of life in a skilled nursing facility setting. The role involves assessing patients, developing and modifying treatment plans, and collaborating with an interdisciplinary team including nursing, therapy, and medical staff. SLPs also participate in community health initiatives and ensure compliance with all legal and organizational standards.

Responsibilities

  • Establish, assess, and modify patient goals based on individual needs
  • Chart progress accurately and timely, utilizing therapy software
  • Oversee and evaluate care provided by Speech Therapy Clinical Fellows (CCC only)
  • Follow treatment plans under supervision (CFY only)
  • Provide excellent patient service and maintain professional relationships
  • Assist in emergency procedures and ensure patient safety
  • Participate in team communication and multidisciplinary care planning

Qualifications & Skills

  • Graduate of an accredited speech therapy program (MS or MA)
  • ASHA certification (CCC) or eligibility for a Clinical Fellowship Year (CFY)
  • Active licensure in applicable state, maintained throughout employment
  • Preferred: 1+ year experience in post-acute or related care
  • CPR certification (must be current)
  • Proficiency in Microsoft Office and communication skills
  • Knowledge of legal, regulatory, and safety standards in healthcare
  • Strong decision-making, body mechanics, and confidentiality adherence
