Speech Language Pathologist / SLP

About the role

Flagstar Rehab is staffing full‑time Speech Language Pathologists for the rehab department in state‑of‑the‑art skilled nursing facilities throughout NYC and surrounding areas. SLPs work with a diverse, multi‑disciplinary team to perform evaluations, treatments, and patient/family education to improve speech and swallow capabilities and daily functioning. The position offers both direct employment (W‑2) and independent‑contractor (1099) options, with assistance setting up a corporation free‑of‑charge.

Responsibilities

• Provide efficient speech or swallow evaluations and/or screens of patients
• Develop plan of care, downgrade/upgrade as appropriate, manage meal texture
• Provide quality therapy services to individuals in an SNF setting requiring care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnoses
• Monitor and document patient progress using innovative EMR software
• Proficiency in Rehab Optima + Point Click is a plus

Requirements

• Not accepting Clinical Fellows (CFY) at this time
• Current NYS SLP License and Registration
• Clinical Competence Certification (CCC)
• CPR Certificate
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)

Pay

$46‑$48 hourly (W‑2), $57‑$60 hourly (1099)
