Responsibilities

• Promote Elara Caring’s philosophy, mission statement, and administrative policies.
• Evaluate patients to determine rehabilitation needs, potential for achievement, and develop a plan of care.
• Admit patients and assess speech-language therapy needs according to OASIS and PPS regulatory guidelines.
• Make initial therapy evaluation visits and re-evaluate patient needs during each visit.
• Initiate plans for patient safety using patient, family, and community resources.
• Communicate significant findings, problems, or changes in condition to supervisors, physicians, and other team members.
• Participate in the implementation and development of the Plan of Care to ensure quality, continuity of care, and proper discharge planning.
• Verify the Plan of Care prior to each visit and provide care according to physician’s orders, assessment data, and established standards.
• Write physician orders for additional visits and changes to the Plan of Care, per agency policy.
• Incorporate patient care goals into therapy care and revise the Plan of Care in response to identified issues.
• Perform direct Speech-Language pathology services in accordance with accepted standards and physician certification.
• Select diagnostic and therapeutic materials appropriate to patient needs.
• Instruct patients and caregivers in home therapy programs, providing modifications and clarifications as needed.
• Ensure clinical notes reflect ongoing communication and coordination with physicians and other staff.
• Submit accurate therapy notes, supply logs, and visit logs according to office policy.

This role operates in a physically demanding, high-stress environment with a full range of body motion, including handling, lifting, and transferring patients. You may work irregular hours, including on-call shifts.

Requirements

• Master’s degree from an accredited educational program in Speech-Language Pathology.
• Current, unrestricted SLP license in the state of work.
• Minimum one (1) year of experience as an SLP in a clinical care setting.
• Able and willing to travel within the branch/office coverage area.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
• Must be able and willing to travel up to 50% of the time.

You will report to the Branch Director, Clinical Manager, Clinical Supervisor, or designee.
