How You'll Work
- Provide home-based skilled speech therapy care for patients as directed by the attending physician
- Educate patients and their family members and ensure the safety of the patient
- Plan of Care: Develop an individualized ST plan of care and make adjustments as needs change
- Guide the patient in his/her use of therapeutic, creative, and self-care activities for the purpose of improving functioning
- Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff
- Patient Outcomes: Use activities and home exercise programs to improve swallowing, breathing, cognition, speech, and communication, and/or any other speech therapy related function
- Minimize Patient Risk: Help decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions
- Family Educator/Advocate: Counsel patient and family on the disease/injury and how to manage in all phases of speech therapy
- Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines
- Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors
- Attend in-service training and mandatory agency meetings
Requirements
- Current license in the state of employment
- Minimum one-year experience as a ST in an acute care setting
- Home care experience preferred
- Valid driver's license and auto insurance in your name as a driver
- Capable of all physical demands
