Responsibilities
- Evaluates and treats patients after provider referral, developing and implementing a plan of care based on evaluation findings and appropriate treatment techniques.
- Carries out the plan of care with reassessment and makes changes as necessary.
- Performs and interprets instrumental exams and hearing screens as needed.
- Therapist is responsible for meeting the productivity expectations of their department.
- Plans and helps coordinate discharge plans with the multidisciplinary team.
- Instructs patients and caregivers/families, recommends diets/instrumentals for discharge, and communicates with physicians, nurses, case managers, social workers, and other team members.
- Involved in the multidisciplinary patient plan of care and discharge readiness plan, documenting evaluations, progress toward goals, discharge plans/continuity of care, and other involved medical and insurance parties.
- Supervises patient assignments and duties of the rehabilitation tech, as applicable.
- Participates in department quality improvement through data collection and team meetings.
- Assumes duties delegated by management in the absence of department management.
Qualifications
- Master's Degree in Speech Pathology
- Licensed in the State of Ohio
- Basic computer skills
- Specialized knowledge in cognition, aphasia, dysphagia, video fluoroscopic, and endoscopic swallowing experience (for Home Care department)
- Valid state driver’s license, minimum of eighteen (18) years old, acceptable MVR, and proof of insurance, if applicable
