Brief Description

AuthoraCare Collective is seeking a PRN Speech-Language Pathologist Case Manager for its Home Health department serving Alamance/Rockingham Counties. The position requires travel to and functioning in patients' homes, providing therapy services, assessments, and rehabilitation plans.

Responsibilities

  • Provide speech or language therapy services according to the patient’s plan of care.
  • Develop and implement a home exercise plan tailored to each patient.
  • Conduct comprehensive assessments covering cognition, medication management, swallowing, speech, and provide educational and rehabilitative treatment plans.
  • Collaborate with the interdisciplinary team to manage the speech therapy plan for integrated health patients.
  • Assess patient needs, set therapy goals, and apply various methods such as exercises, hands-on therapy, and equipment.
  • Work with physicians to establish a treatment plan and maintain a therapeutic relationship with patients.
  • Document therapy goals and outcomes.
  • Perform evaluations and treatments for hospice patients as necessary.

Requirements

  • Masters or doctoral degree in speech-language pathology.
  • Minimum one year of speech therapy experience in home health or facility setting preferred.
  • Current and unrestricted Speech Therapy License.
  • CPR certification.
  • Valid state-issued driver's license required.
  • Own transportation.
