Responsibilities
- Provide high-quality therapeutic interventions to patients in SNFs (Skilled Nursing Facilities)
- Collaborate with interdisciplinary teams to develop and implement comprehensive care plans
- Conduct outcome tracking and analysis to measure patient progress and service effectiveness
- Ensure compliance with regulatory standards and participate in Quality Assurance activities
- Manage risk assessments and develop customized rehabilitation plans tailored to individual patient needs
- Handle appeals and prepare for state surveys with professionalism and accuracy
Requirements
- Bachelor's degree in Physical Therapy, Occupational Therapy, Speech-Language Pathology, or related field
- Current licensure to practice in your state
- Experience in a healthcare setting, preferably in a SNF environment
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
