About the Role
You’ll be at the heart of our community, providing compassionate and personalized therapy services to clients in their homes.
Responsibilities
- Evaluate clients by obtaining and interpreting subjective and objective data
- Develop personalized treatment plans with short-term and long-term goals, ensuring they align with physician orders
- Maintain high standards of care while adapting to different environments and client needs
- Document all services accurately and promptly using Vivie’s EMR system
- Demonstrate an understanding of all payer sources and authorization requirements
- Complete and document supervisory visits with HHAs as per regulatory guidelines to ensure cohesive and compliant care
- Recommend and oversee continuation programs to support long-term client success and optimize discharge outcomes
- Make referrals to continuation programs as necessary, ensuring clients receive the appropriate follow-up
- Maintain clear and compassionate communication with clients, families, referral sources, physicians, and the interdisciplinary team
- Utilize exceptional verbal and written communication skills to ensure cohesive and effective care delivery
- Ensure regulatory compliance by completing and documenting supervisory visits with RA/CNAs in accordance with state and regulatory guidelines
Requirements
- Current Speech Language Pathologist (SLP) license in the state of Minnesota
- CPR certification within 6 months of hire
- Valid driver's license and access to an insured personal vehicle
- Ability to pass state mandated background check
Qualifications
- Strong clinical skills in speech therapy rehabilitation
- Knowledge of evidence-based speech and language therapy practices
- Proficiency in reading, writing, and speaking English to communicate effectively with clients, families, and team members
- Physical capability to perform job duties
- Willingness to float across regions as needed
- Experience in home health, long-term care, or rehabilitation settings preferred
