Speech Language Pathologist (In House SNF) Casual PRN

About the role

Evolve Therapy Services is hiring a Speech Language Pathologist for an in-house Skilled Nursing Facility in Madison, WI. This is a PRN position. The role involves planning, organizing, developing, and directing Speech Language Pathology services in accordance with federal, state, and local standards to ensure the highest quality of resident care.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate, following established standards of practice, therapy department procedures, and productivity standards.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop treatment plans that include a target list, short-term goals, and long-term goals.
• Instruct patients, families, and staff in appropriate speech-language procedures to allow carryover of therapeutic remediation and goals into daily activities.
• Complete compliant and medically necessary documentation per CMS expectations in NetHealth EMR systems.
• Oversee and implement proper protocols for FEES/MBS with coordination of site programs.
• Participate in screening protocols to ensure residents' needs are met timely and with clinical excellence.

Requirements

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency (CCC).
• Active SLP license in the state(s) of practice (must have Wisconsin license).
• Ongoing continuing education and professional development to maintain current licensure and certification.
