Speech Language Pathologist

Responsibilities

  • Evaluate residents with speech, language, cognitive-communication, voice, and swallowing disorders.
  • Develop and implement individualized treatment plans to maximize functional outcomes.
  • Provide skilled speech therapy services to short-term rehabilitation and long-term care residents.
  • Perform dysphagia assessments and implement appropriate swallowing interventions.
  • Collaborate with physicians, nursing staff, dietitians, and the interdisciplinary care team to ensure optimal resident outcomes.
  • Educate residents, families, and caregivers regarding treatment plans and communication strategies.
  • Complete accurate and timely documentation using Net Health and PointClickCare (PCC).
  • Maintain compliance with all federal, state, and facility regulations.

Qualifications

  • Current Pennsylvania Speech-Language Pathologist license.
  • Certificate of Clinical Competence (CCC-SLP) required.
  • Master's degree in Speech-Language Pathology from an accredited program.
  • Skilled Nursing Facility (SNF) or Long-Term Care experience preferred, but not required.
  • Experience with Net Health and PointClickCare (PCC) is a plus.
  • Strong clinical assessment, communication, and organizational skills.
  • Passion for providing compassionate, resident-centered care.
