Responsibilities
- Evaluate residents with speech, language, cognitive-communication, voice, and swallowing disorders.
- Develop and implement individualized treatment plans to maximize functional outcomes.
- Provide skilled speech therapy services to short-term rehabilitation and long-term care residents.
- Perform dysphagia assessments and implement appropriate swallowing interventions.
- Collaborate with physicians, nursing staff, dietitians, and the interdisciplinary care team to ensure optimal resident outcomes.
- Educate residents, families, and caregivers regarding treatment plans and communication strategies.
- Complete accurate and timely documentation using Net Health and PointClickCare (PCC).
- Maintain compliance with all federal, state, and facility regulations.
Qualifications
- Current Pennsylvania Speech-Language Pathologist license.
- Certificate of Clinical Competence (CCC-SLP) required.
- Master's degree in Speech-Language Pathology from an accredited program.
- Skilled Nursing Facility (SNF) or Long-Term Care experience preferred, but not required.
- Experience with Net Health and PointClickCare (PCC) is a plus.
- Strong clinical assessment, communication, and organizational skills.
- Passion for providing compassionate, resident-centered care.
