Duties and Responsibilities
- Assess patients referred for speech therapy and rehabilitation services.
- Provide direct patient care by evaluating speech, language, cognitive, and swallowing disorders.
- Develop and implement individualized treatment plans based on physician orders and patient needs.
- Establish realistic, measurable goals and monitor patient progress.
- Complete evaluations, weekly progress notes, recertifications, and discharge documentation.
- Educate patients, families, and caregivers on therapy techniques and treatment plans.
- Collaborate with interdisciplinary team members to provide high-quality, patient-centered care.
- Perform additional duties as assigned by the Director of Rehabilitation.
- Maintain compliance with HIPAA, confidentiality requirements, and all applicable federal and state regulations.
Qualifications
- Graduate of an accredited Speech-Language Pathology program.
- Current, valid state license to practice as a Speech-Language Pathologist.
- Certificate of Clinical Competence (CCC-SLP) preferred, if applicable.
- Long-term care and/or geriatric experience is preferred but not required.
- Strong communication, documentation, and teamwork skills.
