Role: Speech Language Pathologist providing in-home therapy to seniors and adults with chronic conditions. Key Responsibilities Assess speech, language, cognition, and swallowing functions.Develop personalized, evidence-based treatment plans.Deliver therapeutic interventions to improve communication and swallowing abilities.Educate patients and caregivers to promote independence at home.Collaborate with an interdisciplinary team to coordinate care.Document patient care in compliance with home health regulations.Contribute to a quality, patient-centered, and growth-oriented environment.
