Role: Speech Language Pathologist (SLP) providing personalized care to older adults in various settings, including private homes and senior living communities. The role involves conducting evaluations, developing individualized treatment plans, delivering evidence-based therapy, and supporting patient independence and mobility. Responsibilities Offer one-on-one speech therapy in diverse environmentsPerform assessments and create tailored care plansDeliver therapy to enhance wellness, longevity, and functional independenceDocument in electronic medical records per standardsCollaborate with interdisciplinary team members (PTs, OTs, Fitness Specialists)Educate patients, caregivers, and staff on safety and aging in placeUtilize telehealth/virtual care to extend services and flexibility Qualifications & Skills Valid state licensure or eligibility in Speech Language PathologyDegree from an accredited SLP programProficiency in computers and EMR systemsCurrent CPR certificationASHA CCC certification Physical Requirements: Ability to perform tasks involving moderate physical effort, patient transfers, repositioning, and driving to patient sites; must wear PPE when needed.
