We are seeking a licensed Speech Language Pathologist to deliver personalized, one-on-one care to older adults in private homes and senior living communities, utilizing our Geriatric House Calls™ model. Key Responsibilities Include Providing evaluations, developing individualized treatment plans, and administering evidence-based therapy to support patient mobility, independence, and wellness.Documenting care accurately within electronic medical records and collaborating with an interdisciplinary team of healthcare professionals.Educating patients, caregivers, and staff to promote safety and aging in place.Utilizing telehealth and virtual care to enhance access and flexibility. Qualifications Valid state licensure or eligibility as a Speech Language Pathologist.Degree from an accredited program, current CPR certification, and ASHA CCC.Proficiency with computers and EMR systems. Physical requirements: Ability to perform patient transfers, repositioning, and extended walking, as well as driving between locations and wearing PPE as needed.
