Responsibilities
- Conduct evaluations, develop individualized treatment plans, and deliver evidence-based therapy sessions to support mobility, independence, and wellness.
- Document patient progress accurately in electronic medical records.
- Collaborate with a multidisciplinary team including PTs, OTs, and fitness specialists.
- Educate patients, caregivers, and staff to promote safety and aging in place.
- Utilize telehealth and virtual care to enhance access and flexibility.
Qualifications & Skills
- Valid state licensure or eligibility in Speech Language Pathology.
- Degree from an accredited program and current CCC from ASHA.
- Proficiency in computer systems and EMR software.
- CPR certification.
Physical & Other Requirements
- Ability to perform moderate physical effort, assist with patient transfers, repositioning, and operate a vehicle for home visits.
- Must wear PPE when necessary.
