Role: Speech Language Pathologist providing personalized care to older adults in private homes and senior living communities. Responsibilities Conduct evaluations, develop individualized treatment plans, and deliver evidence-based therapy sessions to support mobility, independence, and wellness.Document patient progress accurately in electronic medical records.Collaborate with a multidisciplinary team including PTs, OTs, and fitness specialists.Educate patients, caregivers, and staff to promote safety and aging in place.Utilize telehealth and virtual care to enhance access and flexibility. Qualifications & Skills Valid state licensure or eligibility in Speech Language Pathology.Degree from an accredited program and current CCC from ASHA.Proficiency in computer systems and EMR software.CPR certification. Physical & Other Requirements: Ability to perform moderate physical effort, assist with patient transfers, repositioning, and operate a vehicle for home visits. Must wear PPE when necessary.
