About the Role
InnovAge is seeking a Speech Language Pathologist to provide clinical speech therapy evaluation and treatment for program participants on an acute, restorative, and/or maintenance level in accordance with InnovAge policies and regulatory guidelines.
Responsibilities
- Provide clinical speech therapy evaluation and treatment for participants.
- Participate in in-service programs and present as assigned.
- Collaborate with healthcare personnel in patient care planning.
- Complete initial history and evaluation visits, notify physicians of patient needs, and submit plans of care for approval.
- Perform all skilled Speech Language Pathology services as ordered by the physician.
- Consult with physicians regarding changes in treatment.
- Write reports to physicians regarding patient progress.
- Attend staff meetings and patient care conferences as scheduled.
- Complete discharge summaries upon completion of therapy services.
- Instruct patients and families in home programs with documentation in medical records.
- Observe confidentiality and safeguard all patient-related information.
- Prepare and submit patient evaluations, clinical, and progress notes in a timely manner.
- Report patient incidents, variances, or complaints to the Director of Clinical Services/Supervisor immediately.
- Participate in peer review and quality management activities as requested.
- Participate in utilization review of medical records as assigned.
- Adhere to Agency standards and perform all assigned responsibilities accurately.
- Report incomplete work assignments to the Director of Clinical Services/Nursing Supervisor/Therapy Supervisor.
- Perform therapy services at clients' places of residence.
- Collaborate with nursing home and assisted living staff regarding participants' speech therapy needs.
- Develop speech maintenance programs for participants to be carried out by Rehab Aides.
- Administer treatment programs per referrals from interdisciplinary team assessments.
- Act as a primary consultant for communication devices and clinical evaluation of swallowing and diet issues.
- Collaborate with dietary and day center staff on participant swallowing issues and appropriate diet textures.
- Conduct and participate in clinic educational programs.
- Perform department record-keeping of speech therapy services and evaluate participant progress using InnovAge PACE documentation requirements.
- Collect data, prepare reports, and maintain participant statistics.
- Maintain quality assurance, safety, environmental, and infection control programs; recommend policies and procedures related to speech therapy.
- Perform other duties as assigned.
Requirements
- Master’s degree in Speech and Language Pathology from an accredited college/university.
- Clinical Certification of Competency in Speech Pathology (CCC).
- 1+ year working with the frail or elderly (personal or volunteer experience applicable).
- 2+ years of healthcare experience in nursing homes and/or the home health industry.
- Knowledge of the scopes or practices of Speech/Language Pathology as it relates to geriatrics.

Pay
Posted pay range: $41.73 - $54.28 per hour.
