About the role

Flagstar Rehab SLPs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the speech and swallow capabilities of their patients and improve ability to function in their daily lives. We offer the opportunity for Direct Employment with our agency or to Be Your Own Boss as an Independent Contractor. We can assist you with setting up a corporation free-of-charge. Ask us how!

Responsibilities

• Provide efficient speech or swallow evaluations and/or screens of patients
• Develop plan of care, downgrade/upgrade as appropriate, manage meal texture
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson's, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnoses
• Monitor and document patient progress with the use of innovative EMR software

Pay

• Hourly Pay Rate: $42-$48 Hourly (W-2), $55-$61 Hourly (1099)

Requirements

• Not accepting Clinical Fellows (CFY) at this time
• Current NYS SLP License and Registration
• Clinical Competence Certification (CCC)
• CPR Certificate
• EMR Competence
• State Required Medical Documentations for work in a Skilled Nursing Facility (Physical, TB Test, MMRV Vaccine Record or Titers, Covid Vaccine Documentation)
• Valid Proof of Identity and Work Authorization
• Malpractice/Liability Insurance (if applicable)
