Actify Home Care is a clinician‑owned agency led by a physical therapist with over 20 years of experience. We are expanding our team of geriatric speech‑language pathologists (SLP) to serve home‑health patients in North Palm Beach County, including Jupiter, Palm Beach Gardens, Riviera Beach, Juno Beach, and surrounding areas.

Compensation

Per‑Visit Compensation (W‑2 employee):

• Evaluation / Re‑Admission: $100.00
• Routine Visit: $80.00

Responsibilities

• Provide high‑quality, one‑on‑one speech, cognitive, and swallowing therapy to geriatric patients in their homes, focusing on dysphagia, stroke recovery, and cognitive‑linguistic care.
• Complete evaluations, re‑assessments, and OASIS documentation (Start of Care, Resumption of Care, and Discharge).
• Develop and implement plans of care to address speech, language, and dysphagia (swallowing) disorders.

Requirements

• Active Florida Speech‑Language Pathologist (SLP) license.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.
• Kinnser/WellSky experience is a major plus.
