About the role
Actify Home Care is growing, and looking for Geriatric SLP home health therapists to join our team in Central Palm Beach County: West Palm Beach, Wellington, Royal Palm Beach, Riviera Beach, Lake Worth and surrounding areas.
Pay
- Evaluation / Re-Readmission: $100.00
- Routine Visit: $80.00
We pay set flat rates per completed visit, to respect your time and travel. Reliable direct deposit bi-weekly, processed automatically based on your completed Kinnser visit logs—no invoice tracking required.

Responsibilities
- Provide high-quality, one-on-one speech, cognitive, and swallowing therapy to geriatric patients in their homes (focusing on dysphagia, stroke recovery, and cognitive-linguistic care). We are open to skilled clinicians looking to transition their foundational SLP experience into adult home health.
- Complete Evaluations, Re-Assessments, and OASIS documentation (Start of Care, Resumption of Care, and Discharge).
- Develop and implement plans of care to address speech, language, and Dysphagia (swallowing) disorders.
Requirements
- Active Florida Speech-Language Pathologist (SLP) License.
- Current CPR/BLS Certification.
- Valid Driver’s License and reliable transportation.
- Kinnser/WellSky experience is a major plus.
