Actify Home Care is a clinician-owned and operated home health agency looking for Geriatric SLP home health therapists to join our team in North Palm Beach County: Jupiter, Palm Beach Gardens, Riviera Beach, Juno Beach, and surrounding areas.

Compensation

SLP Per-Visit Compensation (W-2 Employee):

• Evaluation / Re-Admission: $100.00
• Routine Visit: $80.00

Responsibilities

• Provide high-quality, one-on-one speech, cognitive, and swallowing therapy to geriatric patients in their homes (focusing on dysphagia, stroke recovery, and cognitive-linguistic care). We are open to skilled clinicians looking to transition their foundational SLP experience into adult home health.
• Complete Evaluations, Re-Assessments, and OASIS documentation (Start of Care, Resumption of Care, and Discharge).
• Develop and implement plans of care to address speech, language, and Dysphagia (swallowing) disorders.

Requirements

• License: Active Florida Speech-Language Pathologist (SLP) License.
• Certification: Current CPR/BLS Certification.
• Transportation: Valid Driver's License and reliable transportation.
• Software: Kinnser/WellSky experience is a major plus!
