About the role

The role involves the development, implementation, coordination, and evaluation of clinical programs and services for designated patient populations. The incumbent provides evaluation and direct patient care that promotes optimal health.

Responsibilities

  • Provides general patient care under the direction of or in consultation with the Chief of the Division.
  • Evaluates patient’s physical, emotional, and psychosocial status, and makes recommendations for treatment or referral as appropriate.
  • Independently provides nursing services to patient/family/significant other, including education, counseling, medication management, and general health care instructions.
  • Performs the triage process (telephone and in person) to assist patient in obtaining timely and appropriate care.
  • Consults with others on the health care team as necessary. Responds to appropriate medical emergencies within the department.
  • Acts as a liaison between patient, family, provider, medical facility, or other agencies involved with the patient.
  • Convenes with the attending physicians, residents etc., to ensure delivery of optimum quality of patient care.
  • Participates in special projects as assigned.
  • Prepares and helps Attending Physician performing Fine Needle Aspiration (FNA) of thyroid nodule. Gives appointment to patient for FNA and coordinates the timing with the Ultrasound Department.
  • Participates in meetings with the physicians and other team members to discuss progress and/or management of specific patients.
  • Performs other duties pursuant to clinical responsibility, such as arranging follow-up for patients, obtaining informed consent from patient, recalling patients for further evaluation and treatment, escorting patients, when necessary, to other areas of the hospital, etc.
  • Informs patients of necessary post-discharge instructions as required. Makes appropriate entries in patients’ charts.
  • Demonstrates professional involvement by attending and participating in seminars, workshops and courses as presenter or author; keeping abreast of current literature and trends in practice; considering ethical issues in professional nursing practice.
  • Works with Clinical Diabetes Educators in support group sessions for Diabetes patients.
  • Collaborates with the Clinical Research Coordinator on research protocols, entering data and following up on patients participating in clinical studies.

Qualifications

  • Completion of NYS-approved program for training of registered professional nurses and an accredited program in Nurse Practitioner.
  • Master’s Degree and certification in specialty area preferred.
  • Two years progressive job related experience specific to area of responsibility.
  • Registration required with New York State as Registered Professional Nurse and Nurse Practitioner.
  • Certification by ANCC in specialty area.
  • High level of analytical ability required to effectively and expeditiously diagnose, monitor and deliver care to patients under the direction of physician.
  • Interpersonal skills sufficient in order to effectively interact with patients to gain trust and cooperation.
  • Interacts with various internal hospital medical, administrative and professional staff in the course of routine clinical duties.
  • High degree of responsibility required for contacts with patients and their families.
  • Manual dexterity required for handling of medical equipment physical stamina and mental health necessary to coordinate a broad variety of services and programs for designated patient population on a daily basis.

Compensation

The salary range for the role is $87692 - $131538 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need.
