Responsibilities
- Counsels and screens applicants for program admission.
- Determines eligibility based on agency criteria and reports findings to supervisors.
- Enters all relevant screening and follow-up contacts into the agency’s database.
- Ensures all required documentation is completed and processed according to policy.
- Provides referrals to applicants in need of immediate community-based assistance.
- Follows up with assigned applicants on the eligibility/wait list to expedite their program admission or referrals.
- Analyzes monthly admissions data regarding census and referral sources.
- Conducts outreach and educational services to the local community, schools, and agencies.
- Facilitates psycho-educational lectures and therapeutic groups related to chemical dependency.
- Serves as a liaison and advocate between patients, their families, legal representatives, and community-based agencies.
Qualifications
- High School Diploma or GED.
- Bilingual in Spanish.
- Has at least one (1) year of experience conducting substance use screenings/intake or substance abuse counseling, preferably within a Medication-Assisted Treatment program.
- Has good working knowledge of community social service resources.
- Can clearly articulate the agency’s eligibility criteria and treatment continuum of care.
- Has experience working with individuals who have co-occurring disorders.
- Is knowledgeable about client rights and the Code of Ethics.
- Is proficient in using Microsoft Office Suite and Electronic Health Records (EHR).
- Has experience working with diverse populations, including racial, ethnic, and socioeconomic backgrounds.
