About the role

The South Shore Public Health Collaborative is seeking a Community Health Worker to provide resource navigation, community outreach, partnership building, and public health education to residents within the collaborative, which includes Cohasset, Hingham, and Scituate. The position is part-time and fully grant-funded through June 2027.

The Community Health Worker connects individuals and families with health, social service, and government resources while building partnerships with community organizations and municipal stakeholders to support community health and well-being. This position works collaboratively with residents, municipal departments, regional partners, social workers, nurses, and community organizations.

Responsibilities

  • Conduct regional outreach, community engagement, and public health education activities
  • Build and maintain partnerships with community organizations, municipal departments, and other stakeholders
  • Serve as a liaison between residents, municipal departments, community organizations, and regional partners
  • Develop and maintain a regional resource guide and ensure resource information remains current and accessible
  • Develop relevant program and resource materials and flyers
  • Participate in data collection, program evaluation, community engagement, and public health education initiatives
  • Coordinate meetings, training, community events, and special projects
  • Provide case management services and resource navigation to residents, primarily individuals ages 18–60
  • Connect individuals and families with government benefits, community programs, housing resources, fuel assistance, food resources, and other available services
  • Maintain accurate, appropriate, and confidential records related to case management, referrals, outreach, and program activities
  • Provide regular progress reports and updates to the Coalition Advisory Board during monthly meetings
  • Perform other related duties and responsibilities as assigned

Qualifications

Minimum Qualifications:

  • Bachelor's degree in social work, public health, or a related field
  • Two to three years of professional work experience in social work or public health services
  • Part-time work, internships, and volunteer experience may be considered toward the professional work experience requirement

Preferred Qualifications:

  • LMHC/LCSW preferred
  • Experience working in a municipal, community health, social services, or public health environment
  • Experience with case management, community outreach, resource navigation, public health education, or prevention programs
  • Experience working collaboratively with community organizations and municipal or regional partners
  • Knowledge of local, state, and federal government benefits and community resources

Pay & Schedule

  • Part-time, 15-19 hours per week, 3 days per week
  • Grade A4, $38.10-$40.43 per hour
