Social Worker

Responsibilities

• Oversight of the Social Services Department
• Development, implementation, supervision, and evaluation of medically-related social services
• Identification and provision of medically-related social services in compliance with regulations
• Completion of social services component of comprehensive assessments and contribution to care plans
• Facilitation of residents' transition back into the community through discharge planning
• Aid in locating and accessing community resources for residents
• Interpretation and reporting of residents' social, psychological, and emotional needs
• Documentation of social service actions and interactions in medical records
• Advocacy for residents and assistance in asserting their rights
• Assistance with facility policy development and annual reviews
• Reporting suspected abuse, neglect, or exploitation to appropriate authorities
• Ensuring access to appropriate treatment and resources for residents with mental illness or psychosocial difficulties

Requirements

• Bachelor's degree in social work or another human services field
• One year of supervised social work experience in a healthcare setting
• Strong communication and interpersonal skills
• Ability to work in a fast-paced environment and prioritize tasks
• Knowledge of State and Federal regulations related to social work in healthcare settings
• Confidentiality and handling of sensitive information
• Strong problem-solving and critical thinking skills

Work Environment

This position will be based at Berlin Health & Rehabilitation Center in Barre, Vermont. The Social Worker will work closely with the Social Services Director, medical staff, and other resident care team members.
