About the role
The Socio Therapist provides trauma-informed therapeutic case management and support to children, youth, and families involved in foster care. This role partners with families to develop individualized service plans, promotes safety and permanency, provides crisis intervention, coordinates care with internal and community providers, implements evidence-based practices, and advocates for youth through court appearances, family meetings, and ongoing case management.
Primary Responsibilities And Essential Functions Of Position
- Create, in partnership with families, plans addressing safety, behavioral, psychological, and permanent needs.
- Monitor safety, well-being, and permanency progress through regular casework contacts and provision of necessary services.
- Timely documentation of all reports and casework activities, including FASPS, Psychosocial, Adoption Home studies, Kinship Home studies, and court reports.
- Participate actively in Family Court Hearings, Family Team Conferences, and other meetings to advance permanency for children and families.
- Provide crisis intervention as needed.
- Implement evidence-based models with foster parents, parents, discharge resources, and youth.
- Cookordinate care with other agency and community-based programs such as Behavioral Health & Wellness and Health Homes.
- Provide family planning counseling to youth and document discussions with foster parents.
- Work to identify family resources and support for youth through Family Finding evidence-based practice.
- Frequently communicate with clients to provide services and answer inquiries.
Qualifications
- Bachelor's Degree required; BSW preferred.
- Experience supporting families and adolescents with social services.
- A commitment to working from a strength-based, youth development, and trauma-informed perspective.
- Ability to work with diverse populations of clients and team members.
- Highly organized with the ability to meet program deliverables and deadlines.
- Excellent verbal and written communication skills.
- Flexible to work flexible hours - some evenings or early mornings as needed to meet client needs.
- Ability to use public transportation to travel throughout New York City or Agency vehicle when available.
