Responsibilities
- Weigh in on medical, physical, social, and emotional assessments and Plans of Care.
- Oversight of direct social work case management services for up to 50 LIFE Center members.
- Provide social service support including screenings, counseling, and external referrals, as needed.
- Reevaluate and update Plans of Care based on change in members’ status, emergent conditions, and other relevant circumstances, then communicate changes to all relevant stakeholders.
- Complete ongoing periodic assessments to ensure a thorough evolution of member care, in keeping with changing needs and requests.
- Document all visits, including deviations from approved care plans and prescribed intervention techniques.
- Document member medical records and maintain electronic case management assessments for immediate and future use.
- Identify development areas, including individual member enhancements and overarching LIFE Center opportunities for growth and expansion, then develop and implement these improvements.
- Counsel families in both individual and group settings, and lead relevant activities to educate these care participants.
- Visit in-home members, assess needs, and develop/implement comprehensive speech/language care plans accordingly.
- Perform general administrative duties, including completing and submitting 1768 documents to required agencies.
Qualifications
- Bachelor’s degree in Social Work or related field.
- Current license to practice Social Work in the state.
- At least 2 years of experience in a home health or similar setting.
- Experience working with seniors and their families.
- Excellent interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Proficiency in Microsoft Office Suite.

- Knowledge of Medicare and Medicaid regulations.
