Requirements
- Bachelor's degree in Social Work or a related field
Responsibilities
- Assist in assessments, including Admission Assessments, Trauma assessments, and other evaluations as needed
- Attend Care plan meetings and contribute to the development of individualized care plans
- Affiliate in discharge planning, ensuring a smooth transition for our residents
- Collaborate with the Director of Social Services to address resident needs and concerns
- Support residents and families with emotional and psychological challenges
- Maintain accurate and detailed records of assessments, care plans, and resident interactions
