Responsibilities
- Schedules for WOW employees include every weekend (Nights - Friday & Saturday, Days - Saturday & Sunday).
- Earns premium pay in addition to competitive hourly rates and shift differential.
- Collaborates directly with the care team, care management team and patient, and/or family, and/or the patient’s representative to assess the patient’s continuum of care needs and ensure the patients transition to the least restrictive care environment.
- Assesses the patient’s psychosocial and continuum of care needs.
- Interventions may include, but are not limited to, family support, crisis management, consultations with a staff physician, Adult or Child Protective Services reporting/coordination, custody/guardianship issue coordination, indigent patient program coordination, substance-use support coordination, intimate partner violence support, bereavement.
- Adheres to Norton Healthcare policies and applies educational and professional expertise in the delivery of quality care.
Qualifications
- One year social work or One year social work internship.
- Master Degree in Social Work from accredited programKentucky - Certified Social Worker (CSW), or Indiana - Licensed Social Worker (LSW)
- Desired: One year in a healthcare setting
- Licensed Clinical Social Worker (LCSW)
