Responsibilities
The Social Worker is a member of the multidisciplinary team. The Social Worker collaborates directly with the care team, care management team and patient, and/or family, and/or the patient’s representative to assess the patient’s continuum of care needs and ensure the patients transition to the least restrictive care environment.
- Assess the patient’s psychosocial and continuum of care needs as an independent professional.
- Provide interventions including, but not limited to:
- Family support
- Crisis management
- Consultations with staff physicians
- Adult or Child Protective Services reporting/coordination
- Custody/guardianship issue coordination
- Indigent patient program coordination
- Substance-use support coordination
- Intimate partner violence support
- Bereavement
- Make referrals and coordinate services such as acute rehabilitation, extended care, durable medical equipment, home health, and other resources as necessary.
- Adhere to all Norton Healthcare policies and apply educational and professional expertise in the delivery of quality care.
Qualifications
Required
- One year of social work experience or one year of social work internship.
- Hospital and/or payer credentialing completed before functioning in this role.
- Master’s Degree in Social Work from an accredited program.
- Kentucky - Certified Social Worker (CSW), or Indiana - Licensed Social Worker (LSW).
Desired
- One year in a healthcare setting.
- Licensed Clinical Social Worker (LCSW).
