Social Worker, Clinical Resource Management, Days

Responsibilities

The Social Worker is a member of the multidisciplinary team. The Social Worker collaborates directly with the care team, care management team and patient, and/or family, and/or the patient’s representative to assess the patient’s continuum of care needs and ensure the patients transition to the least restrictive care environment.

• Assess the patient’s psychosocial and continuum of care needs as an independent professional.
• Provide interventions including, but not limited to:
  • Family support
  • Crisis management
  • Consultations with staff physicians
  • Adult or Child Protective Services reporting/coordination
  • Custody/guardianship issue coordination
  • Indigent patient program coordination
  • Substance-use support coordination
  • Intimate partner violence support
  • Bereavement
• Make referrals and coordinate services such as acute rehabilitation, extended care, durable medical equipment, home health, and other resources as necessary.
• Adhere to all Norton Healthcare policies and apply educational and professional expertise in the delivery of quality care.

Qualifications

Required

• One year of social work experience or one year of social work internship.
• Hospital and/or payer credentialing completed before functioning in this role.
• Master’s Degree in Social Work from an accredited program.
• Kentucky - Certified Social Worker (CSW), or Indiana - Licensed Social Worker (LSW).

Desired

• One year in a healthcare setting.
• Licensed Clinical Social Worker (LCSW).
